Please be aware, the family practices of Dr. Lew and Dr. Allaye-Chan are at full capacity, and they are not currently accepting new patients into their practices.
For patients already registered at our clinic, if you are booking/signing up for the first time, please click "Log In" at the top right of this page, and Sign Up by submitting your FULL NAME as it appears on your Care Card. Once we locate your patient chart and confirm that you are a patient registered with either Dr. Lew or Dr. Allaye-Chan, your account will be activated. It may take up to 24 hours to receive a reply to your sign-up application.

REMINDER DUE TO COVID-19 PRECAUTIONS:

If you are experiencing any VIRAL SYMPTOMS (eg: cough, runny nose, loss of smell, sore throat, fever, difficulty breathing, headache, muscle aches, fatigue, loss of appetite, vomiting, or diarrhea), and are booked for an in-office visit, please call ahead to notify our front desk staff.
All patients and family members coming into the clinic are encouraged to wear a mask, especially if experiencing any of the above viral symptoms. We also ask that ONLY the patient and/or their essential support person (eg: translator, parent of a child) enter the clinic. All others may wait outside the clinic.

Dr. Louise Allaye-Chan will remain your Family Doctor. Please book an appointment with Dr. Allaye-Chan via our online booking system or by phone.

PLEASE NOTE:
Dr. Allaye-Chan is not currently accepting new family practice patients. Patients are encouraged to book appointments primarily with their own family doctor.

DR. JASON LEW'S RICHMOND OFFICE:
Oak Tree Family Medicine has been in operation since July 2019. Medical records for Dr. Lew's active patients have already been transfered over to our clinic.
PLEASE NOTE:
Due to practice capacity constraints, for patients whose medical records have not already been transfered over, we are no longer able to maintain your status as an active patient of Dr. Lew's.
For these patients, they will need to find a new family doctor. Medical records for these patients remain located at Richmond-Instore Medical Clinic.
